Print and complete the required information, attach photograph and verification of current certification, and have the Affidavit completed and notarized. For Emergency Licensing: Print and complete the application and temporary work permit, attach required documents, scan everything into a pdf file and email as an attachment to firstname.lastname@example.org Payments are to be mailed separately. An incomplete application will not be processed and will be returned.
This application is for use only by applicants recently graduated, or about to graduate, from an accredited program in Radiography, Nuclear Medicine, or Radiation Therapy who have not yet taken and passed the certification exam. Print and complete the required information, attach photograph, have your Program Director verify and sign, finally have the Affidavit completed and notarized. Send completed application with payment to the LSRTBE office.
This application is to be used for those that are licensed, and certified in Nuclear Medicine by the ARRT or NMTCB, but are not licensed and certified as a Radiographer. The PET scan must be performed by the licensed Nuclear Medicine Technologist, the CT scan must be performed by the licensed General Radiographer, two categories of operators are required. The LSRTBE considers the hybrid machines to be no different than two separate machines. The Temporary Work Permit will allow the licensed and certified Nuclear Medicine Technologist to work toward the ARRT-CT certification. Once that is achieved the Fusion license will be issued.
